Abstract


Objective: To determine the colorectal cancer and liver metastases in patients with tumor-infiltrating lymphocytes distribution, explore tumor infiltrating the lymphatic immune cells circumstances with colorectal liver metastases prognosis; explore the impact of colorectal cancer patients with liver metastases prognosis factors. Subjects and Methods: select our hospital abdominal surgical abdominal surgery admitted to our hospital from January 2006 to December 2007 75 cases of colorectal cancer patients with liver metastases, primary tumor underwent surgical resection achieved pathologically confirmed by CT or B ultrasound or MRI or intraoperative exploration confirmed liver metastases. Measurement OUTCOME MEASURES: selected patients with the complete cases collection and follow-up, follow-up endpoint was death in patients, follow-up deadline in April 2009. Application of mature immunohistochemical staining techniques, the determination of patient primary tumor periphery stove specimens and some metastases specimens, and the the tumor central part of tumor-infiltrating lymphocytes (tumor infiltrating lymphocyte, TIL) CD3 and CD8 expression contrast TIL CD3, CD8 the expression and parts, to study tumor-infiltrating lymphocytes distribution, as well as TILCD3 CD8 positive expression and prognosis relations. Results: 1.75 cases of colorectal liver metastases in patients lost to follow five cases, the dropout rate was 6.7%, the shortest follow-up in March, up to 36 months, the median survival time was 16 months, and the 1-year survival rate was 57.8%, 2 year survival rate was 42.7%. Colorectal cancer patients with liver metastases, tumor surrounding tumor infiltrating lymphocytes CD3 positive rate of 53.3% and the CD8 positive rate of 25.3%; the tumor central tumor-infiltrating lymphocytes CD3 positive rate of 48%, the CD8 positive rate of 21.3%. 3. Tumor surrounding area infiltrating lymphocytes CD3 expression impact on prognosis (p lt; 0.05), tumor the Chuo-infiltrating lymphocytes CD3 expression, no statistically significant influence on the prognosis infiltrating lymphocytes CD8 expression; whether the tumor metastases resection grouping, tumor-infiltrating lymphocytes CD3 expression on patient survival difference (p lt; 0.05). Conclusion: In patients with colorectal liver metastases, surgical radical excision of the lesion is still to cure the most effective way to improve survival. Both liver metastases or metachronous liver metastases, such as non-surgical contraindications, metastases resection, should seek a surgical resection, such as stage resection difficult, can be a palliative resection underwent two surgical adjuvant therapy Radical resection.
