Abstract


[ Objective] To observe high mobility protein -1 expression in severe acute pancreatitis combined with somatostatin and growth hormone levels after treatment to explore the high mobility protein -1 in severe acute pancreatitis and clinical treatment efficacy evaluation . [Method ] The experimental part of the male SD rats were retrograde cholangiopancreatography injection of 3.5% sodium taurocholate 2.5ml/kg injection pressure to maintain the 3.4 - 3.9kPa made ??SAP rat model . The rats were randomly divided into six groups of 40 : (1) sham-operated group ; ( 2 ) SAP group ; (3) saline group ( SAP NS ) ; ( 4 ) somatostatin ( SAP S ) ; ( 5 ) the somatotropin group ( SAP G ) ; ( 6 ) the combined treatment group ( SAP SG ) . The inspection of each group 24 hours after rat pancreatic tissue HMGB - 1 , endotoxin , TNF - alpha , IL-6 , IL - 1beta level changes ; while observing the terminal ileum TEM under the ultrastructural changes and pancreas pathology score . Huashan Hospital from June 2005 to December 2009 hospitalized patients with severe acute clinical part of the collection is divided into the the somatostatin treatment group ( n = 11 ) and growth hormone , somatostatin combined treatment group ( n = 10 ) , observation and analysis of HMGB - 1 and SAP patients complicated with MODS, the relationship of bacterial translocation ; the dual hormone therapy HMGB - 1 levels in patients with SAP . [ Results ] The delay increased HMGB - 1 in a rat model of acute pancreatitis compared with the TNF - alpha and other inflammatory mediators . Growth hormone combined with somatostatin therapy can reduce the levels of endotoxin , TNF - alpha , IL - 1 , IL - 6 level ; reduce tissue inflammation of the pancreas ; SAP rat intestinal pathological change mitigation , increased villus height and mucosal thickness , improve SAP survival rate of rats . Pancreatic tissue HMGB- 1 mRNA expression decreased , consistent with these changes . Clinical data showed patients with SAP MODS, the bacterial translocation plasma HMGB - 1 level was significantly higher than that in the control group ; plasma HMGB - 1 concentrations decreased growth hormone combined with somatostatin treatment of SAP . [ Conclusion ] SAP rat pancreatic tissue HMGB - 1 mRNA expression in their serum endotoxin , TNF - alpha , IL - 1 , IL - 6 , and pancreas pathological changes in trends . Plasma HMGB - 1 can predict the SAP patients mods , the occurrence of bacterial translocation ; evaluation of growth hormone combined with somatostatin treatment effect of SAP .
